The Role
As a Permanent full-time Clinical Care Manager, you will manage the care of residents in our facility. This role reports directly to the Residential Service Manager and ensures residents receive safe, high-quality care aligned with our model and philosophy, the Aged Care Quality Standards, regulatory requirements, and professional practice guidelines.
* Educate and mentor care staff to deliver high-quality care, supporting them to achieve excellence in their roles.
* Support the admission of new residents, including assessments, care planning, and reviews, to ensure seamless transitions.
* Manage complex healthcare issues, such as wound care, infection control, pain management, falls, and continence issues, promoting optimal health outcomes.
* Consult with residents, their families, allied health, and medical practitioners to provide comprehensive care and support.
* Lead the accurate and timely completion of reporting and documentation in line with relevant legislation, regulatory requirements, professional standards, policies, and procedures.
Selection Criteria
To be successful in this role, you will need:
* At least two years of experience in a care management role in residential aged care, demonstrating a deep understanding of care delivery.
* A passion for delivering person-centred care, prioritizing the needs and preferences of each individual resident.
* Excellent communication skills, written, verbal, and interpersonal, enabling effective collaboration and stakeholder engagement.
* Ability to work autonomously, taking initiative and responsibility for achieving excellent outcomes.
* Registration with AHPRA as a registered nurse is essential, ensuring the highest level of clinical competence.
* Hold and maintain NDIS Worker Checks (or willing to obtain), upholding the required standards and expectations.

Hours: Full-time; 76 hours per fortnight
Salary: $120K to $140K, based on relevant experience and subject to approval + 12% Superannuation + NFP salary packaging
