Your new role: 
As a Senior Social Worker, you will deliver advanced social work services to patients and families in GEMRHITH and Day Therapy services at QEII Jubilee Hospital. You will support complex care needs, contribute to evidence-based practice, and provide leadership in clinical education, supervision, and service development. 
Key responsibilities include:
- Delivering evidence-based social work assessments, interventions, and discharge planning
- Applying advanced clinical judgement to address complex psychosocial needs
- Providing supervision, education, and support to HP3 staff, students, and AHAs
- Contributing to quality improvement, service innovation, and research initiatives
- Liaising with patients, families, carers, and community services to ensure safe transitions of care
- Supporting interdisciplinary team processes and patient-centred care planning
About you: 
We are seeking a compassionate and motivated senior clinician who thrives in rehabilitation and aged care environments. 
To succeed in this role, you will demonstrate: 
- Tertiary qualifications in Social Work and eligibility for AASW membership
- Advanced knowledge of social work practice within rehabilitation and geriatric care
- Strong clinical skills in assessment, intervention, and discharge planning
- Experience in mentoring, supervision, and interprofessional collaboration
- Proven ability to lead service improvement and contribute to evidence-based practice
- Excellent communication, negotiation, and conflict resolution skills

The Queen Elizabeth II Jubilee Hospital (QEII) is a medium metropolitan adult facility providing a range of inpatient and outpatient services including a 24-hour Emergency Department. We provide a range of services, including surgery, cardiology, endoscopy, orthopaedics, gynaecology, urology, geriatric, respiratory and emergency medicine.
