Your Next Role
9D Care is looking for a Support Coordinator / Psychosocial Recovery Coach in Perth to join our team on a Permanent Part-Time basis (approx. 15 to 20 hours/week). This position is fully work-from-home, supporting existing NDIS participants in the Perth area. You’ll help participants understand their plans, connect with the right supports, and make well-informed choices.
This role is expected to convert to full-time employment as our growing participant base in WA continues to expand — we’re already receiving frequent referrals. We hope to convert this to a Full Time Role as soon as we have enough billable work and participants to fulfill it so we would appreciate someone who has the availability and willingness to do Full Time work soon.
Key Responsibilities
- Coordinate day-to-day operations of NDIS Support Coordination and/or Psychosocial Recovery Coaching.
- Support participants to understand and implement their NDIS plans, goals, and objectives.
- Connect participants with informal, mainstream, community, and funded supports.
- Advocate for participants’ rights and ensure choice and control in their decision-making.
- Monitor, review, and report on progress toward participant goals.
- Maintain accurate records in line with NDIS operational guidelines.
- Maintain Case Notes and billable hours on our NDIS Software MYP.
- Regularly complete reports as required by the NDIS for each participant.
- Help NDIS Participants prepare for their plan reviews and submit all evidence required for plan reviews.
What We Need From You
- Working knowledge of the NDIS or relevant experience in disability, community, or mental health sectors.
- Certificate III or higher in disability services, community services, mental health, or relevant health/psychosocial qualifications.
- Experience working alongside people with disability, their families, and carers.
- Knowledge of the NDIS operational guidelines, service navigation, and case management principles.
- Strong written and verbal communication skills.
- Competence with software systems for participant data entry and tracking plan funding.
- Current Australian driver’s licence.
- Relevant checks: Working With Children Check & NDIS Worker Screening Check (or willingness to obtain).
- Eligibility to work in Australia.
