The Pathways to Community Living Initiative (PCLI) is a small, specialist consultation-liaison service based at Citigate Mental Health Centre in North Gosford. This team works together to find practical, creative solutions that are tailored to each person's needs.
Role Description
This role provides expert guidance—rather than case management—to clinical teams across the district who work with adults experiencing severe and persistent mental illness. The focus is on recovery-oriented, person-centred care that supports improved service culture and reduced hospital length of stay.
The ideal candidate will have current AHPRA registration as a Registered Nurse, Psychologist or Occupational Therapist or eligibility for membership with AASW for Social Workers. They must also have demonstrated ability to work effectively and collaboratively both independently and as part of the multidisciplinary health care team.
Key Responsibilities
* Provide complex care and develop and implement new model of care partnerships for individuals with severe and persistent mental illness, complex care needs, and alcohol or other drug use.
