We are expanding our Community Connection Support Officer team to meet a growing need in our communities. This is your opportunity to support people to live more connected, independent, and fulfilling lives.

Your Role
">
You will work directly with clients to reduce social isolation and strengthen their links to both informal and formal supports. You will be the bridge between individuals and the services that can transform their lives – whether it's health, housing, education, or social engagement.

Key Responsibilities:
">
* Collaborate with Service Leaders to identify client goals and community opportunities.
">
* Support clients in accessing health, mental health, and wellbeing services.
">
* Help clients navigate funding pathways such as NDIS, Aged Care, and Child and Family Connect.
">
* Connect clients with local groups, employment services, education providers, and more.
">
* Maintain a local database of community services and ensure accurate client records.
">
* Prepare discharge plans that ensure sustainable support networks.

What We're Looking For
">
A passion for community connection and social inclusion is essential for this role. You will ideally have a Certificate IV in Community Services, Disability, or Mental Health. Experience supporting clients in a community-based or case management role is also highly desirable.
">
Strong communication skills and ability to build trust quickly are crucial. You should be highly organised with strong time management and IT skills. A commitment to safeguarding and upholding Centacare's values of dignity, inclusion, and respect is also required.
