About The Service

The primary role of AARCS (Acute to Aged Related Services) is to assist older individuals—and their carers and families—to access appropriate services to support a safe and timely discharge from the hospital. The service is dedicated to improving assessment, care planning, and discharge planning for older people with complex needs in the inpatient setting.

Key Responsibilities


* Provide specialist care to inpatients aged 65+ with complex care needs.
* Facilitate coordinated, timely discharges by linking patients and families with appropriate community services.
* Advocate for patients and their families, ensuring that their needs and preferences are heard and respected.
* Collaborate closely with social workers, external agencies, ACAT teams, and community aged care organisations.
* Communicate and negotiate effectively with a wide range of stakeholders.

About You

We're looking for someone with solid experience in acute care settings, who knows how to navigate the complexities of working with ACAT teams and community aged care organisations. You'll need to be a confident communicator, skilled negotiator, and passionate advocate for older patients and their families. If you thrive in a multidisciplinary team environment, love building strong partnerships, and bring a compassionate, organised, and patient-centred approach to everything you do — this could be the perfect role for you.

* The successful candidate will hold a current registration as a Registered Nurse with AHPRA
