Clinical Care Coordinator Role
As a Clinical Care Coordinator, you will be responsible for delivering high-quality clinical services to clients. Your role will involve liaising with internal and external stakeholders, providing evidence-based interventions, and maintaining accurate records.
* Liaise with various stakeholders to establish partnerships and ensure effective service delivery.
* Direct the provision of clinical services, including intake, assessment, and referral, as well as complex case management and coordination.
* Risk assess and intervene in a timely manner to ensure client safety.
* Provide individual, couple, and family counselling based on evidence-informed practices.
* Analyse client data and prepare reports as required to inform service improvement.
* Work systemically with key stakeholders to influence recovery approaches within a stepped-care model for vulnerable or at-risk veteran and defence force communities and families.
* Participate in community development activities to promote services and enhance service reach.
* Contribute to the design and delivery of group programs to support client needs.
About You
To be successful in this role, you will need to possess specific skills and qualifications. These include:
* A registration as a Registered Mental Health Nurse, Psychologist, Social Worker, or Occupational Therapist.
* Full registration with AHPRA or AASW.
* Working with Children (Blue Card) or Working with Vulnerable People (Yellow Card).
* Australian citizenship and willingness to undertake a National Police Clearance.
