About the Role
Centrecare seeks a compassionate Registered Nurse with experience in mental health to join our team.
* Delivering clinical assessments and supporting medication management in collaboration with General Practitioners
* Providing ongoing care coordination and treatment planning for clients including comprehensive assessments
* Offering education and support to clients and their families through evidence-based interventions that promote wellbeing and recovery
* Facilitating referrals and shared care arrangements with external services to ensure continuity of care
* Maintaining accurate client records using electronic databases such as Penelope
