At St Pat's, we walk alongside families experiencing homelessness and crisis. Our Family Support Program provides specialised, assertive support to improve safety and stability until sustainable housing is secured.
We are seeking a passionate and experienced Child & Adolescent Assertive Case Worker to provide outreach-based, trauma-informed and culturally safe support to children and young people (0–17 years) experiencing or at risk of homelessness.
This is a unique opportunity to ensure children's voices, safety, and developmental needs are prioritised within whole-of-family interventions.
About the Role
Working alongside our Assertive Case Managers, you will:
* Deliver intensive outreach case management to children and adolescents living in highly vulnerable circumstances (including rough sleeping, unsafe accommodation, FDV, AOD exposure and entrenched poverty).
* Conduct comprehensive child-centred assessments covering safety, wellbeing, development, education, health and trauma needs.
* Develop and review strengths-based, child-focused care plans that centre children's rights and participation.
* Advocate within education, health, disability, child protection and community service systems.
* Provide crisis response and ongoing developmental and wellbeing support.
* Work collaboratively in a co-case management model to strengthen whole-of-family outcomes.
* Maintain high-quality documentation aligned with Child Safe Standards and funding requirements.
This role requires flexibility, initiative and confidence working assertively in outreach settings.
About You
You are:
* Qualified in Social Work, Psychology, Community Services or a related child-focused field.
* Experienced in case management with children and young people, ideally within homelessness, housing or family services.
* Skilled in engaging children and adolescents in age-appropriate, trauma-informed and culturally responsive ways.
* Knowledgeable about the impacts of homelessness, family violence, poverty and trauma on child development.
* Confident working proactively in complex, high-risk environments.
* Committed to child safeguarding, cultural safety and best practice frameworks.
* Strong in documentation, advocacy and communication.
Experience working with Aboriginal and Torres Strait Islander children and families is highly valued.
