About us:
The Transition Care Program (TCP) provides short-term care that seeks to optimise functioning and independence of older people after a hospital stay. TCP is goal-orientated, time-limited and therapy focused. It provides older people with packages of services that include low intensity therapy, such as physiotherapy and occupational therapy as well as social work, nursing support or personal care. It seeks to enable older people to return home after a hospital stay rather than enter residential care prematurely.
About the role:
The Administration Officer works within the Transition Care Program (TCP) in community settings according to Commonwealth guidelines and the Aged Care Act 1997, supporting the TCP Clinicians within the multi-disciplinary team to provide rehabilitation therapy to frail aged clients post an acute hospital stay. In particular, this position supports the Personal Care Workers in their role of providing care/services to the clients while they are on the TCP program.
About you:
Below-mentioned certificates/qualifications would be well regarded for this role but are not a mandatory requirement:Certificate III in Home Care or Aged Care or equivalent work experience.Certificate IV in Health Services Assistance / Allied Health Assistance.What we offer:
