Care Coordinator Role
Anglicare North Queensland is a not-for-profit human services organisation providing place-based initiatives.
The role involves coordinating care for children and young people under statutory orders, ensuring access to required supports and living arrangements that are stable, safe, nurturing, and affirming of culture.
A support plan will be developed, implemented, and reviewed in collaboration with each child and young person. The plan should be informed by their strengths, needs, preferences, and aspirations.
To be successful, you will need:
* Tertiary qualifications in Social Sciences or equivalent experience combined with 5 years' relevant experience.
* Strong record of working with children/young people from diverse cultural backgrounds who have experienced trauma and complex care needs.
* Ability to maintain professional boundaries and apply positive behaviour strategies, relational approaches.
* Prior experience in case management/casework/support coordination/navigating service systems.
Requirements include holding a current QLD driver's licence, Blue Card, national Police Check, CPR & First Aid certificate, and being willing to provide two reference checks.
