Community Services and Development Support Worker
This role is a unique opportunity to make a difference in the lives of individuals with mental health and alcohol and other drug (AOD) needs.
About the Role
* Conduct initial assessments and referrals to determine the recommended level of care
* Facilitate reviewing and recording of participant outcomes and coordinate navigation, linkage, care, and discharge planning
* Provide mental health and AOD education and intervention within a dual diagnosis framework
You will be working as part of a multidisciplinary team to deliver a comprehensive service that meets the needs of consumers. Your expertise will be utilized to develop and implement individualized care plans, ensuring seamless integration between local, area, and wellbeing services.
Required Skills and Qualifications
* Relevant degree or substantial experience in psychology, case management, social work, mental health, or AOD
* Proven customer service skills, including developing and delivering solutions to consumers, carers, and families
* Knowledgeable in accessing a range of community resources, particularly in the areas of health, drugs, and alcohol, legal, income support, employment, and recreation
