The successful candidate will work closely with the care team, residents, families, and Facility Manager to drive high-quality outcomes through evidence-based assessments and thoughtful care strategies.
Key Responsibilities
1. Conduct AN-ACC Assessments in collaboration with care staff and leadership.
2. Design and implement best-practice care plans that reflect individual needs and values.
3. Undertake Resident of the Day (ROD) reviews alongside the Facility Manager.
4. Ensure assessments are completed within 72 hours of admission and updated as required.
5. Consult with residents, families, Div 1 nurses, and external providers to guide care planning.
6. Lead continuous improvement initiatives through audits and feedback loops.
7. Support carers and deliver emotional, social, and clinical support to residents.
8. Manage specialised care areas including mobility, nutrition, wound care, pressure care, continence etc.
9. Maintain thorough documentation and communication across care teams and families.
10. Complete HELF Checks and coordinate required training with PAL and P&C for new staff.
