Anglicare Southern Queensland's Home and Community service offers a range of support and assistance to enable our older population to live their best lives in their own homes. This full-time opportunity is for a Team Leader to join our Toowoomba Home and Community Service.
About the role:
The Team Leader will support the delivery of community services to an assigned group of clients and will assist in the facilitation and monitoring of individualised care plans which meet the needs of clients as delegated. The Team Leader will supervise Home Care Workers and Support Services Workers on a day-to-day basis, assist with orientation of new staff and provide guidance and support including assessments, client related documentation and other related activities as delegated.

Facilitate and monitor individualised care plans to meet client needs as delegated.
Support the delivery of community services to an assigned group of clients.
Qualifications and experience
Previous experience in a senior care worker or supervisory role with experience facilitating non-clinical community-based training.
Able to lead, manage, train and mentor team members.
Able to assist with planning and managing client care.
Possess highly developed interpersonal skills.
Demonstrated time management skills.
Current National Police Certificate.
Current Driver's Licence and own reliable vehicle.
Certificate IV in Home and Community Care or equivalent.
Current First Aid and CPR Certification.
