We are seeking a casual female support worker to provide Community Social and Recreational Activities to one of our clients in Leongatha.
* Provide direct support to NDIS participant in line with their individual care plans.
* Assist with daily living activities, including personal care, meal preparation, and medication reminders.
* Support clients in accessing social and community activities to promote inclusion.
* Provide transport assistance for appointments, shopping, and recreational activities.
* Maintain accurate and timely documentation, including progress notes and incident reports.
* Follow NDIS guidelines, company policies, and procedures to ensure compliance.
* Communicate effectively with participants, families, and other stakeholders.
* Uphold workplace health and safety practices to ensure a secure environment for all.
Licence/Certification:
* Certificate III in Disability or Aged Care Services
* First Aid Certification
* NDIS Worker Screening Check
* Driver Licence with own car
* National Police Certificate (Preferred)
* Working with Children Check (Preferred)
