We are seeking compassionate and skilled individuals to work with our Aged Care Assessment program. As a Clinical Assessor, you will be responsible for traveling to clients' homes, learning about their lives, and supporting them through identifying strategies and supports to maintain independence as they age.
This is an exciting opportunity to work with a dynamic team that values innovation, empathy, and purpose. Our extensive training and support enable you to convert client interactions into meaningful support plans documented using digital tools guided by our Active Assessment method.
About the Role
* You will travel to clients' homes and learn about their lives.
* You will support clients in maintaining independence as they age.
* You will identify strategies and supports for clients.
This role requires adaptability, strong communication skills, and empathy. You must be passionate about problem-solving and have a growth mindset. A Bachelor degree qualification in Physio, OT, Social Worker, or RN is required, along with current professional registration at the time of application. Legal eligibility to work in Australia is also essential. At least one year of experience working in aged care or with older people is preferred.
