This is a 6‑month position with guaranteed full‑time hours, a hybrid work model, and the opportunity to make a real difference for older Australians living in the community. Pay can be maximised through NFP salary packaging.
What the role involves
Deliver and develop tailored Care Plans for each client and oversee the ongoing assessment of clients' needs and preferences within Home Care.
Schedule and manage client interactions, ensuring that each plan remains up to date and that clients receive an appropriate type and amount of care, aligned with funding and approval.
Responsibilities
* Assist in providing services tailored to the needs of individual clients, delivered within budget and service expectations.
* Manage care plans and client‑related KPIs for existing clients.
* Ensure care plans are current and that clients receive the right level/type of care.
* Provide professional day‑to‑day delivery of services and support staff in understanding clients' preferences.
Qualifications
* Certificate IV in Aged Care and a minimum of 2 years of relevant industry experience.
* Knowledge of the Home Care Packages Program, Support at Home, Commonwealth Home Support Programme, and Aged Care Quality Standards.
* Experience operating effectively in a multi‑site environment.
* Current driver's licence and willingness to travel between clients, sites, and services (salary package can include a motor vehicle on request).
* Strong administrative and computer skills.
