Primary objective
Ensure delivery of high-quality, culturally appropriate and effective patient navigation within the Emergency Department as part of a youth
patient navigation pilot program. This position may require the applicant to work within an extended hours service model, including
weekends.

Context statement
This position, based within the RDPH Division of Emergency Medicine, is responsible for managing a youth navigation pilot program
alongside a social worker. The program aims to enhance youth-friendly care and patient navigation, focusing on social and emotional
wellbeing, social care needs and optimising access to health and social care in the community. The person is expected to work
autonomously, understands the health system and organisation, and the ability to collaborate with a strong cohesion within the
multidisciplinary team. Training and development opportunities will be supported and negotiated.

Key duties and responsibilities
1. Provide leadership to the multidisciplinary health care team through providing a specialised high level of health care and education to

patients with complex needs to build both cultural and clinical competence.
2. Provide timely and culturally safe care, advice and health education to a patient group with diverse and complex care requirements.
3. Work within a multidisciplinary framework and provide feedback to the senior staff and management on program and clinical issues.
4. Prioritise and manage a caseload within a changing work environment to ensure timelines and health outcomes are met.
5. Engage with community members and key stakeholder organisations to strengthen community involvement in health service delivery.
6. Follow defined service quality standards, work health and safety policies and procedures relating to the work being undertaken in order

to ensure high quality, safe services and workplaces.
7. Foster a positive workplace culture through exemplary work practices and role modelling behavior.

Selection criteria
Essential
1. Current registration with AHPRA as an Aboriginal and/or Torres Strait Islander Health Practitioner with no conditions.
2. Minimum AQF Level 5 -as per NTPS Aboriginal Health Practitioner 2022 - 2025 Enterprise Agreement.
3. Demonstrated experience providing Aboriginal and Torres Strait Islander primary care practice to individuals and families, including care

coordination and health education and promotion.
4. Proven organizational skills and ability to coordinate and manage programs.
5. Well-developed communication, interpersonal skills, negotiation and conflict management skills, both verbal and in writing, to maximize

productive working relationships with internal and external stakeholders.
6. Demonstrated local cultural knowledge, respect and safety at the family and community level.
7. Must be able to provide confirmation of Aboriginality; obtain a criminal history check and clearance; and working with children’s

clearance (Ochre card).

Desirable
1. Experience in mental health/youth work

Further information
Any applicant who does not hold the essential qualification or an identified equivalent qualification will be appointed for a fixed term period
of two years, subject to engaging and participating in a learning agreement to obtain the required qualification. The successful applicant
will be required to provide all essential documents outlined including their Confirmation of Aboriginality prior to being appointed to the
vacancy. Positions may be subject to pre-employment checks such as immunisation requirements, working with children clearance notice
and criminal history checks. A criminal history will not exclude an applicant from this position unless it is a relevant criminal history. For
