The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The focus of IUIH Aged Care team is to work with the local Indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in‐home care services to eligible aged care clients.
About the Role
The primary purpose of this position is, in accordance with the clients' assessed needs, to manage the contract relationship and arrangements with the client/carer, review ongoing service needs, and coordinate a team of care workers in the delivery of the agreed suite of services ensuring quality, timeliness and effectiveness of service delivery. The role will be required to perform in accordance with Client Care plans, the Community Care Common Standards and the IUIH Policies and Procedures.
Responsibilities
* Demonstrate practical and innovative leadership in the design, implementation and evaluation of daily programs for clients
* Liaise with the Assessor and Community Nurses to ascertain client needs
* Identify a range of services that enhance inclusion for elderly Indigenous persons who are residing in their own homes within the community
* Collaborate with other service providers to ensure effective service delivery, and increase community networks
* Coordinate and review service arrangements with Service Providers
* Assist in the planning, implementation, review and evaluation of programs
About You
* Current C Class Driver's Licence (QLD)
* Current Criminal History Check
* Current Disability Worker Screening Check (Yellow Card)
* Current First Aid and CPR Certificates
Desirable, but not mandatory
* Demonstrated experience working with Aboriginal and Torres Strait Islander people and/or communities
