Job Overview
The Motor Neurone Disease (MND) Share Care Worker is a regional role in the Hume Region of Victoria, integral to the Palliative Care team.
* Promotes early referral to palliative care services.
* Supports, develops, and delivers education to palliative care service providers.
* Coordinates between palliative care services, MND clients, and families in the Hume region.
This role not only supports individuals living with MND but also their families, health professionals, and community healthcare providers.
Key Responsibilities
* Assists individuals living with MND in navigating pathways as their care needs change.
* Works closely with the MND Advisor and Support Coordinator.
