Care Navigator Recovery Support Program

This new program is designed to improve patient mental health outcomes and prevent readmissions by providing comprehensive, recovery-oriented outpatient/outreach services following discharge from an inpatient setting.
Key Responsibilities
* Identify, enroll, and plan clinical care for eligible patients, ensuring seamless transition to the least restrictive care.
* Assess patients upon admission, liaise with patients, carers, and clinicians during their inpatient stay, and assist with discharge planning.
* Support patients to access ongoing outpatient/outreach care aligned to their needs.
* Work alongside treating psychiatrists and a multidisciplinary team to coordinate and deliver patient-centred care.
Qualifications
* 5+ years of experience in clinical mental health with a strong understanding of mental health conditions, pathways, and referral processes.
* Excellent clinical assessment, communication, and organisational skills to develop patient-centred care plans and manage patients/families.
* Current AHPRA registration as a Registered Nurse, Mental Health Nurse, or a Mental Health Allied Health professional.

Requirements
* Must provide a National Police check conducted within the previous 12 months.
* May be required to complete a Working With Children check and proof of immunisation against infectious diseases.
