About the Role:
We're seeking a Permanent - Full time Registered Nurse to join our Home and Community Care team.
You'll partner with customers to assess their needs, implement strength-based strategies and evaluate effectiveness.
As a Registered Nurse, you'll work within the broader Home and Community team, including allied health, case managers and community support workers.
We're committed to delivering person-centred care and best practice in relation to the provision of quality care.
Key Responsibilities:
* Assess and review customer needs in consultation with case managers
* Clinical Nursing assessments (PAS & KICKA)
* Cognitive assessments
* Develop, implement, and evaluate plans of care in partnership with customers and their significant others for both chronic and complex needs
* Liaise with appropriate health care professionals to deliver quality care to customers
* Participate in multidisciplinary team meetings
* Ensure full compliance with organisation, professional and aged care standards
* Contribute to the accurate maintenance of funding mechanism and monitor expenditure.
Key Requirements:
* Current registration with AHPRA as a Registered Nurse
* Ability to relate to elderly customers with empathy whilst working effectively with other health professionals, service providers and case managers
* Strong clinical assessment skills
* Work autonomously with strong communication, organisational and time management skills
* Enthusiasm for professional growth and development
* Competency with IDCs, wound care and strategies for living with dementia – desirable
* Cultural awareness and sensitivity
* Current Australian driver's licence and reliable vehicle - both essential
