Lead Aged Care Programs with Heart and Purpose
Empower ageing well in Port Broughton, leading meaningful programs that promote independence, connection, and wellbeing for participants in our aged and disability services.
Key Responsibilities
* Coordinate and deliver Aged Day Programs aligned with industry guidelines, fostering a culture of respect, inclusion, and continuous improvement.
* Lead a team of support workers, promoting client independence and social engagement while maintaining accurate records and quality documentation.
* Liaise with case managers and contribute to care planning, driving service development, evaluation, and improvement.
* Support the delivery of inclusive, engaging programs, leveraging expertise in aged care and community settings.
Requirements
* Completed AQF Certificate III relevant to Aged Care, with experience supporting aged or disabled persons in health/community settings.
* Strong communication, coordination, and problem-solving skills, with the ability to lead a team and drive program success.
* Knowledge of Aged Care Standards, CHSP, HCP, and NDIS, with proficiency in Microsoft Office and administrative processes.
