About the role
The Clinical Nurse Specialist - Palliative Care, delivers complex specialist nursing care and leads the development of specialist clinical practice and service delivery. The position provides effective quality care, for patients with a life limiting illness with complex needs and supports their families/carers to improve the quality of life and wellbeing of individuals and rural communities.
This role requires specific skills and knowledge in palliative care, to allow the provision of a client centred, consultative service for people with an advanced life limiting illness, their families and carers. The provision of this care will occur across a range of settings, including inpatient, home and residential aged care. As a telehealth enable LHD this role will provide care through a range on modalities including face to face, telephone and telehealth.
The successful applicant will deliver complex, specialised nursing care to people with life limiting illness and provide education to build the workforce capability and confidence in managing palliative care patients of Western NSW Local Health District, to improve the health and wellbeing of individuals and rural communities.
