About the Role
We are seeking an experienced and motivated Support Coordinator to join our growing team. This role involves working closely with NDIS participants to help them understand, implement, and maximise their NDIS plans while building capacity, choice, and control.
You will manage a caseload of participants with varying needs and complexity, working collaboratively with families, providers, and other stakeholders to achieve meaningful outcomes.
This role suits someone who is organised, proactive, communicative, and confident managing competing priorities.
Key Responsibilities
* Support participants to understand and implement their NDIS plans
* Build and maintain strong relationships with participants, families, and service providers
* Coordinate services in line with participant goals, preferences, and funding
* Monitor budgets and support participants to use funding effectively
* Identify risks, gaps, and emerging issues and take timely action
* Assist participants to prepare for plan reviews, change of circumstances, and appeals where required
* Maintain accurate, timely, and compliant case notes and documentation
* Work in line with NDIS Practice Standards, organisational policies, and ethical obligations
* Communicate clearly and professionally with all stakeholders
Essential Requirements
* Minimum 12 months experience as an NDIS Support Coordinator (or equivalent relevant experience)
* Strong working knowledge of the NDIS, including reasonable and necessary criteria
* Excellent written and verbal communication skills
* Strong organisational and time-management skills
* Ability to work independently while contributing positively to a team
* High level of professionalism, integrity, and accountability
* Competency using CRM systems and Microsoft 365
* Current NDIS Worker Screening Check
* Current Working With Children Check
* Australian Driver's Licence and reliable vehicle
Desirable (but not essential)
* Experience in Specialist Support Coordination
* Psychosocial or complex needs experience
* Knowledge of SIL, SDA, and behaviour support pathways
* Experience preparing review and change of circumstances documentation
