We're looking for a Service Coordinator to join our team and play a key role in supporting families in the Northern Suburbs (maternity leave cover).

About the role
* Coordinate services and case management for children and families.
* Manage client onboarding, including liaison with families and external providers.
* Complete and maintain all required documentation such as care plans, service agreements and risk assessments.
* Maintain regular client contact, including home visits and ongoing review of supports.
* Work with the rostering team to manage staffing changes and ensure appropriate staff allocation.
* Support staff performance, problem solving, mentoring and participation in recruitment activities.
* Assist the Director Clinical Operations with operational planning, continuous improvement and professional development requirements.
About you
* Experience supporting clients in the disability and/or health sector
* Working knowledge of the NDIS.
* Experience supervising and leading staff.
* Strong analytical and problem ‐ solving skills.
* Highly competent with technology and able to learn new systems quickly
* Strong communication and interpersonal skills
* High attention to detail with a commitment to continuous improvement.
* Ability to work independently, manage priorities and adapt to change.
