What Your Day Will Look Like...
You'll work closely with individuals and families to coordinate care, develop case plans and provide advocacy that helps people navigate the health system with confidence.
You'll visit homes, link people with services, run health education sessions and collaborate with clinicians to ensure care is culturally safe and responsive.
You'll also maintain strong community connections, contribute to cultural events and support staff in understanding the importance of culturally appropriate practice.
