About the role:

* Deliver clinical care in the home environment supporting clients to stay living in their home and connected to their community
* Embody a person-centred approach, recognising and promoting the individual's unique goals and preferences
* Ensure thorough clinical assessment, planning, monitoring, and evaluation of care delivered
* Organise and conduct case conferences and maintain open communications with clients and their families, carers and authorised representatives
* Effectively prioritise client needs in collaboration with other medical and health professionals as required

About You:
* You demonstrate compassion, empathy and advocacy
* Excellent communication skills, both verbal and written
* Ability to work independently and demonstrate sound decision-making
* Ability to work efficiently by optimising the use of technology within your role as well as prioritising your time effectively across your caseload
* Current AHPRA registration and Australian Working Rights
* Current valid driver's license and a licensed, comprehensively insured and road worthy motor vehicle
It is a mandatory requirement for a team member working in the Aged Care sector to have a valid Police Check or a valid National Disability Insurance Scheme (NDIS) check, or potentially both, to undertake the inherent requirements of the role. Clearances must be verified prior to commencing employment with RSL LifeCare.
