Job Title: Disability Support Worker
We are seeking a dedicated and compassionate Disability Support Worker to join our team.
The successful candidate will have a passion for working with individuals with disabilities and delivering person-centred care that promotes independence and empowerment.
The Role:
* Provide daily living support and personal care to clients, either individually or as part of a team.
* Facilitate access to community events and medical appointments, promoting socialisation and connection.
* Maintain accurate documentation to track client progress and health outcomes.
* Develop and implement individualised care plans to meet the unique needs of each client.
About You:
* A strong understanding of the NDIS Practice Standards and a commitment to a person-centred approach.
* Experience in supporting individuals with disabilities, with a focus on promoting their goals and aspirations.
Requirements include flexibility with work hours, including evenings, weekends, and overnights. The ability to transport clients is also essential.
