About the Role
The Support Coordination Team exists to connect participants with an NDIS plan to supports and services. The team is the first point of contact for NDIS participants, families and Support Coordinators who are seeking information about New Horizons or who have chosen us to provide services for them under an NDIS plan.

Key Responsibilities
- Ability to constructively navigate and adapt to change.
- Understanding of Coordination of Supports and Recovery Coaching under the NDIS.
- Working with people with a Psychosocial Disability and demonstrated knowledge of the awareness of relevant disability service standards, Mental Health Act and other legislative requirements.
- Able to develop appropriate relationships, internally and externally associated with the role that benefits both the customer and the organisation.
- Actively seeks and encourages innovative solutions to perform under pressure and maintain resilience.
- Developing creative solutions to overcome day to day challenges.
- Building and maintaining effective relationships.
- Risk management and WH&S; requirements.
- Using Microsoft office and systems for reporting.

What you will bring to the role
- Minimum Cert IV in Mental Health Peer Work, or equivalent.
- Valid NSW Drivers License (Class C), with ability to drive large cars and/or vans.
- Willingness to obtain an Australian Federal Police Check, NSW WWCC and NDISWC to meet New Horizons employment criteria prior to commencing with us.
- A strong set of values that guides your work, incorporating our organisational values of passion, integrity & respect.
- Great computer skills with proven ability to use microsoft office and customer relationships management systems.
