About the Role

This position offers an opportunity to work in a challenging and rewarding environment, supporting Aboriginal people living with chronic conditions. As a Clinical Nurse Consultant, you will be responsible for delivering high-quality patient care, working closely with local GPs and primary health networks to ensure seamless coordination of care.


Job Description

You will spend your days visiting communities across Cessnock, Muswellbrook, Singleton, and Maitland, providing clinical assessments, case management, and care coordination. Your expertise as a nurse will bring comfort and reassurance to patients, empowering them to manage their conditions effectively.

No two days are the same in this role, with a mix of clinical work, community engagement, and collaboration with healthcare professionals. You will have the opportunity to make a real difference in the lives of Aboriginal people, promoting better health outcomes and improving quality of life.


Requirements

* Registered Nurse with current Authority to Practice with AHPRA
* 5 years full-time equivalent post-registration experience, with at least 3 years full-time equivalent experience in this specialty field. In addition, you must have approved postgraduate nursing qualifications in chronic disease management.
