**Social Work Services**
The Social Work team across Monash Health is responsible for the delivery of a broad range of Social Work services for clients across the lifespan in the inpatient and outpatient setting.
**About the Role**
Clayton Social Work Department is looking for an experienced Grade 1 social worker to join the team working across clinical areas such as General Medicine, Respiratory Medicine, Thoracic Surgery, various medical surgical specialty units and with pregnant patients requiring telemetry monitoring at Monash Medical Centre. This full-time, permanent role, is part of a flexible team who collaborate to provide a holistic approach to service delivery. This clinical role provides an opportunity to showcase your skills in working with people from a range of diverse backgrounds as well as completing complex psycho-social assessments and development of patient centred care plans. This role is located within the bed-based team and requires participation in the Emergency Department and on-call rosters. Located at MMC in Clayton, travel is also required across other Monash Health sites.
**About You**
You will be a dynamic, innovative and flexible practitioner with a good knowledge of resources, have a good understanding of referral networks. You will be able to demonstrate skills in psycho-social assessments, problem solving, communication and hospital discharge planning. You are innovative and dynamic in the field of Social Work and you ensure that high quality Social Work services that place the client at the centre are provided. Education, quality improvement and evidence based practices will be your key drivers. Due to your proven clinical experience, you will bring knowledge of healthcare reforms and initiatives and challenges that public healthcare is facing now and in the future.
**What you need**:
- You must be eligible for AASW membership including continuing professional education
- Proven experience in providing social work in acute settings
- Knowledge and experience working closely with an inter-professional teams
- Current Driver's Licence
- Current Working with Children's Check
