The ICU Outreach Nurse acts as a resource for ward medical, nursing and allied health staff in managing adult patients through the acute phase of physiological instability. The ICU Outreach nurse supports staff involved in managing patients with complex care needs, including the deteriorating ward patient.
In addition, the Outreach team respond to referrals from multiple disciplines to assess and make recommendations regarding the management of adult in-patients. The ICU Outreach nurse is required to provide clinical expertise and guidance for the ongoing management of tracheostomy patients.
Key responsibilities in this role include identifying, planning and implementing strategies that promote patient safety while promoting access and flow to the wards with an increase in patient acuity.

**About You**
Selection criteria:
- Must be registered as a Critical Care Registered Nurse in Australia with a current practicing certificate
- old a postgraduate ICU qualification with current ICU experience
- Ability to work independently and within a multidisciplinary team
- Proven Nursing leadership skills
- Demonstrate a high level of communication, interpersonal and conflict resolution skills
- Support and encourage a multidisciplinary team approach to management of the acutely unwell patient
- Establish and maintain effective interpersonal relationships with patients and families involved with Intensive Care Unit
- Undertake informal and formal education of Monash Health employees in relation to the deteriorating patient in liaison with key stakeholders
- Be innovative, resourceful and adaptable to change
- Ability to organise resources
