We are seeking a dedicated Care Manager to join our team in Albury.


About the Role

* As a Care Manager, you will be responsible for coordinating and overseeing personalized home care services for clients in Albury.
* You will manage care plans, build relationships with clients and families, and collaborate with the office team to deliver seamless care.


Key Responsibilities

* Plan and review client care goals to ensure high-quality, person-centred care.
* Communicate proactively with clients, families, and team members to develop and update care plans.
* Meet with clients and families face-to-face to review care plans and ensure needs are being met.


About You

* Previous experience working with seniors or aged care is essential.
* A background in care coordination, case management, or client-facing support roles would be highly regarded.
* You must be able to work under pressure and multitask shifting priorities.
