About War Memorial Hospital
War Memorial Hospital (WMH) is a 3rd Schedule Aged Rehabilitation hospital, located in Waverley. It is owned and operated by Uniting and part of the South Eastern Sydney Local Health District (SESLHD). Services include 24‐hour inpatient care, Monday‐to‐Friday outpatient departments, specialist community teams (Transitional Aged Care, Aged Care Assessment Team, Geriatric Flying Squad, Young Onset Dementia, 7‐day day centre, 24‐hour regional accommodation) and a full range of hospital support services. The hospital is set within beautiful tranquil grounds and operates in a supportive team environment.
Role Summary
The Rehabilitation Clinical Nurse Consultant Grade 2 (CNC2) is a senior clinical leadership position. The role provides advanced consultancy, leadership and expert nursing practice across both the inpatient aged rehabilitation (Morgan Unit) and the Transitional Aged Care Program (TACP). The CNC2 leads clinical excellence across the full patient journey, from inpatient rehabilitation through to community‐based transitional care, supporting complex care coordination, optimal functional outcomes and person‐centred service delivery.
Key Responsibilities
* Provide high‐level clinical guidance to multidisciplinary teams, drive quality improvement initiatives and contribute to strategic planning, research and education aligned with ARNA competencies and NSW Health frameworks.
* Lead clinical supervision, education and service innovation, fostering a culture of excellence, accountability and continuous improvement.
* Represent WMH at local and district forums and support the development of integrated models of care across inpatient and community settings.
* Work closely with the Nurse Unit Manager, TACP Manager and WMH Executive to develop clinical services, build workforce capability and advance evidence‐based practice.
* Facilitate complex care coordination and promote person‐centred service delivery across inpatient and community sites.
Qualifications
* Registered Nurse (Registered with AHPRA) with a minimum of 5 years full‐time equivalent post‐registration experience, including at least 3 years in aged rehabilitation, gerontology, community care or a related specialty.
* Approved postgraduate nursing qualifications relevant to Aged Care, Rehabilitation or Chronic/Complex Care, or a commitment to commence within 12 months.
* Current Class C Driver's Licence (required for community‐based TACP activities).

* Demonstrated advanced clinical leadership, expert clinical reasoning and the ability to deliver, evaluate and guide high‐quality patient care across inpatient and community settings.
* Demonstrated commitment to professional development, capability building and clinical supervision for self and others.
* Proven experience in quality improvement, service development, research translation and the ability to lead multidisciplinary improvement initiatives.
* Demonstrated ability to work autonomously and collaboratively within complex multidisciplinary and cross‐site environments.
* Excellent interpersonal, written and verbal communication skills, including high‐level computer literacy.
* Demonstrated commitment to consumer participation, person‐centred care, and engagement in service planning, evaluation and policy development.
Requirements
* All Category A NSW Health workers must have received an annual seasonal influenza vaccination or have a medically approved contraindication certificate.
* COVID‐19 vaccinations are strongly recommended for all NSW Health workers.
