Our organization seeks a dedicated and compassionate Targeted Care Packages Support Worker to join our team. The ideal candidate will have experience in assisting keyworkers with the emotional and functional needs of young people, including developing personal skills, administrative requirements, and providing practical assistance related to the child or young person's case plan.
Key Responsibilities
* Provide support to young people by responding to their emotional and functional needs
* Develop and enhance personal skills, as well as assist with administrative tasks
* Offer practical assistance related to the child or young person's case plan
* Participate in recreational activities for and with children or young people
* Record and communicate relevant information about the children or young people, families, carers, and lead tenants
The successful candidate will work collaboratively with the child or young person, their family, carers, and others to facilitate changes that support achievement of the child or young person's case plan.
