About the Role
This position offers a unique opportunity to provide high-level input, information, advice, referral, and care planning for aged care patients, families, carers, acute, and community services involved in the care of elderly people with age-related health issues and complex needs.
In addition, the role will be responsible for the provision of aged care assessments of patients in line with MNCLHD Aged Care Assessment Team guidelines.

Requirements
The ideal candidate will have a current registration with the Occupational Therapy Board of Australia; or Bachelor of Social Work and eligible for membership to the Australian Association of Social Workers; or Current Registration with the Psychology Board of Australia; or Current Registration with the Nurses and Midwives Board of Australia; or current registration with the Physiotherapist Board of Australia.
Candidates must demonstrate an ability to use problem-solving skills and a multidisciplinary team approach in the planning, delivery, and coordination of patient care.
