About the role
We have an exciting opportunity for a full-time role with Trio Support Services as a Support At Home Care Coordinator. This role focuses on coordinating services, managing care plans and budgets, and ensuring high-quality, person-centred support under the Support at Home Program which is funded through My Aged Care.
Key responsibilities include:
* Coordinating services and supports under Support at Home
* Intake, assessment, care planning and ongoing reviews
* Liaising with clients, families, service providers and health professionals
* Monitoring service delivery, client outcomes and budgets
* Ensuring compliance with aged care legislation and Quality Standards
* Maintaining accurate client records and documentation
About you:
* Qualifications in Aged Care or Community Services is preferred
* A minimum of 2-3 years experience providing care coordination or case management is preferred
* Knowledge of Support at Home or Home Care Packages
* Strong communication, organisation and stakeholder engagement skills
* Confident working in a changing reform and a compliance driven environment
* Current National Police Check
* Full driver's licence and reliable vehicle
