Job Summary

Provide clinical leadership for the Acute Frailty Team within the Emergency Department (ED), ensuring early identification and management of frailty and geriatric syndromes. The role aims to improve care for older adults presenting to ED, reduce length of stay (LOS), rationalise investigations and medications, and minimise complications during ED stay or inpatient admission.

Support the Department of Health's TEC2 (Time to Emergency Care) targets by streamlining patient flow and facilitating timely access to appropriate care pathways. This includes diversion to services such as inpatient Geriatric Evaluation and Management (GEM),, or community‑based programs, and expediting return to Residential Aged Care Facilities (RACF) with support from RIR (Residential in Reach), HITH (Hospital In the Home), and local GPs.

Educate ED staff on best practice for frailty and delirium management, falls risk mitigation, and person‑centred care. Enhance communication between ED, inpatient teams, community providers, and families to ensure continuity of care and timely updates on progress and plans.


Key Result Areas

* Provide clinical leadership for the Acute Frailty Team to deliver high‑quality, evidence‑based care for older adults in ED.
* Achieve TEC2 performance benchmarks through improved patient flow and timely decision‑making.
* Identify frail older patients suitable for diversion to out‑of‑hospital care.
* Facilitate safe discharge and ongoing community care, reducing unnecessary inpatient admissions.
* Commence discharge planning early for patients with frailty admitted to hospital.
* Collaborate with allied health practitioners, pharmacists, medical and nursing colleagues to optimise community‑based care pathways.
* Implement strategies that shorten LOS for geriatric patients while maintaining quality and safety.


Responsibilities

* Provide clinical leadership for the Acute Frailty Team within Monash Health Emergency Departments.
* Contribute to excellence in clinical services, research, education, and knowledge transfer in geriatric and frailty care.
* Foster a multidisciplinary team culture with open communication and collaborative working relationships.
* Support junior medical staff and allied health professionals in delivering evidence‑based frailty care.
* Support continuous improvement initiatives to enhance patient flow and meet strategic objectives for geriatric emergency care.


Person Specification

* Motivated to work in a high‑paced environment.
* Commitment to a team‑based approach to achieve strategic objectives.
* Previous ED Geriatric experience an advantage.
* Flexibility to work across Monash Health Emergency Departments.
* Availability for weekend work as required.


Qualifications / Registrations / Licences

* FRACP (Geriatrics) or eligibility for specialist registration prior to commencement.
* Advanced knowledge of geriatric syndromes, frailty assessment tools (e.g., Clinical Frailty Scale), and evidence‑based interventions for older adults.
* Strong commitment to multidisciplinary collaboration and continuous quality improvement in emergency and acute care environments.
* Familiarity with community‑based care pathways such as GEM,, RIR, HITH, and RACF discharge planning.
