The Opportunity
The role of the Care Team Leader is to lead the team of Community Care staff who provide direct care to our clients. This is a permanent full-time position working Monday-Friday, located in the South East Sydney Home Care Team.
About the role
The role ensures that staff have a good understanding of individual client needs and preferences and can provide services tailored to the needs of individual clients, delivered within budget and service expectations.
Responsibilities
Leading, supporting, and mentoring their team, and working in partnership and consultation with the Care Manager, Clinical Care Manager, Client Liaison Manager, and Regional Area Manager, where required.
Managing the recruitment of new Care Workers and ensuring all working validations, educational and development plans and resources are available.
Performance coaching and managing a team while operating in a multi-site environment.
Traveling between HammondCare's clients, sites, and services, maintaining a current driver's licence and safe driving record.
Strong organisational, administrative and computer skills.
Empathy and patience.
Qualifications
Certificate IV in Aged Care (desirable), HR or similar degree & a minimum of 2 years relevant industry experience.
Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme.
