Crowley Care has an exciting opportunity for an experienced and motivated individual to join our Home Care Team as a Case Manager.

The Case Manager will work in partnership with clients to plan, develop, and implement care support plans, ensuring package delivery optimises client's safety, health, and wellbeing and overall quality of life whilst promoting independence through holistic, preventative and reablement approaches.

Successful candidates will have relevant qualifications and demonstrated experience providing high quality, consumer directed case management, with an understanding of how to manage individualised funds to achieve this aim.

Key responsibilities will include:

* Utilising case management principles and processes to provide tailored, person-centred care and plans with careful consideration of the client's individual needs, goals, and preferences.
* Preparing and reviewing client budgets and actively working with the client to ensure that these are understood, and to ensure supports provided remain within budget constraints without diluting the quality of services.
* Managing an allocated client caseload, conducting assessments and reviews customised to ensure a holistic approach to meet the client's individual situation.
* Developing and maintaining a balanced partnership with existing and perspective home care clients / families.
* Liaising with and promoting Home Care services to external providers.

To be successful in the position you will have:

* Must have a minimum of a Certificate III in Individual Support (Ageing); other relevant qualifications will also be considered.
* Must have demonstrated and relevant experience in case management. Aged care case management experience is highly preferred.
* Demonstrated ability to create and implement holistic and person-centred client directed services.
* Proficient computer skills in Microsoft Word, Excel and Outlook and the ability to confidently learn and utilise organisational software and client management systems.
* Proven strong communication, well developed interpersonal skills.
* Strong business acumen with demonstrated experience in budgeting and accountability reporting.
* The understanding of how case management contributes to increasing capacity, improving sustainability and improving outcomes for clients.
* A current driver's license.
