Within a highly structured and integrated clinical model, you will support patients with complex chronic conditions including pain, mental health, and long-term care coordination, all backed by strong clinical, nursing, and administrative support.

Working as part of an experienced multidisciplinary team, you will provide comprehensive GP care to a dedicated veteran patient cohort, including:
* Chronic disease management
* Pain and musculoskeletal care
* Mental health assessment and treatment planning
* DVA documentation and care coordination
* Ongoing continuity of care via clinic and telehealth
You will become the primary treating clinician, guiding each patient's long-term healthcare journey.

ABOUT YOU
* FRACGP or FACRRM qualified
* Holding current AHPRA registration
* Committed to high clinical governance and documentation standards
* Interested in chronic disease, pain, mental health, and Veteran healthcare
* Collaborative and comfortable working within a multidisciplinary model
* A TGA Authorised Prescriber (or willing to obtain)
* Previous DVA experience is advantageous but not essential
