Primary objective
Promote and support the relationship between NT Health, NT Primary Health Network (PHN) and Primary Care providers in the NT. The
role is primarily located within Big Rivers Region and is responsible for pro-actively engaging with all sectors of Big Rivers Region Primary
Care to strategically improve communication, collaboration and integration between primary care and tertiary care with the focus on
person-centred care.
Context statement
This role works collaboratively across the strategic health reform agenda through connections with NT PHN, Primary care and KDH clinical
divisions. At a strategic level the role works closely with its Top End and Central Australian GPLO counterparts to ensure NT wide system
alignment. At an operational level the role spends 1 day per week equivalent leading operational activities to improve the engagement
and intersection of primary and tertiary care.
Key duties and responsibilities
1. Provide medical leadership within KDH for discussions about regional primary care capacity.
2. Develop and implement health pathways and collaborative models of care that improve the referral process, system navigation and
the transition of patient care between primary and hospital health care. Support, sustain and develop capacity within KDH for ongoing
patient safety and clinical quality improvement.
1. Provide advice to the GPLO steering committee and executives in relation to programs and integrated care activities.
2. Working with the NTPHN clinical education team; identify learning gaps and emerging issues affecting engagement and integration
activities and develop initiatives and pathways targeted at these gaps and issues to improve the transition of care between primary
and hospital care settings.
1. Undertake clinical work as required to ensure up to date expert knowledge.
2. Follow defined service quality standards, work health and safety policies and procedures relating to the work being undertaken in
order to ensure high quality, safe services and workplaces. Foster a positive workplace culture through exemplary leadership practices
and role modelling behaviour.
Selection criteria
Essential
1. Registration with the Medical Board of Australia with, or be eligible for, Fellowship of ACCRM or RACGP.
2. Demonstrated high level interpersonal, verbal and written communication skills.
3. Demonstrated clinical experience, leadership and supervision of clinical quality improvement activities with a commitment to
outstanding patient centred care within a multidisciplinary environment.
4. Demonstrated ability to lead and foster teamwork including managing conflict and negotiate effectively.
5. Excellent problem-solving skills including innovative, conceptual and analytical abilities.
6. Demonstrated ability to work under pressure and time constraints to achieve required goals and tasks.
