The Role
The Clinician – (End of Life) coordinates multidisciplinary care and provides clinical oversight for clients at the end of life. You'll partner with clients, families, and internal teams to deliver compassionate, person-centred support that aligns with each client's needs and goals. You'll also mentor and guide team leaders and care partners, ensuring safe practice, timely escalation, and compliance with best practice.
This is an office-based, phone and email role with no shift work. You'll build meaningful connections with clients, families, and colleagues, using emotional resilience and advanced communication skills. You'll also be supported with structured supervision, facilitated debriefings, and EAP access.
What You'll Do
End of Life Care Management
* Undertake comprehensive clinical and psychosocial assessments
* Collaborate with clients and their families to develop and review End-of-Life Support Plans, integrating palliative care principles and person-centred goals.
* Coordinate timely access to multidisciplinary supports (e.g. palliative specialists, equipment, allied health).
* Ensure smooth transitions into and out of the pathway as needs change, including planning for bereavement and post-death support.
Clinical Supervision & Risk Management
* Provide clinical advice, mentoring, and supervision to care partners and team leaders.
* Escalate risks and support incident management in line with governance protocols.
* Monitor adherence to clinical standards and documentation requirements.
* Facilitate reflective debriefings and promote staff wellbeing after client death or complex cases.
Client Empowerment & Communication
* Promote choice, dignity of risk, and supported decision-making throughout the pathway.
* Inform clients, carers, and families about end-of-life options and supports.
* Facilitate care conferences and communication for coordinated care delivery.
* Provide advanced communication and counselling to support families through grief and decision-making.
* Respect cultural and spiritual preferences in end-of-life planning.
Collaboration, Compliance & Quality
* Foster collaboration through reflective practice, multidisciplinary meetings, and sharing up-to-date palliative care knowledge
* Support integration of the End-of-Life Pathway and maintain strong stakeholder relationships to enable effective care management
* Ensure adherence to all applicable aged care legislation, including the Aged Care Act 1997, Aged Care Quality Standards, and the Home Care Package Program
* Provide expert guidance on clinical matters, quality assurance, and regulatory compliance
What You'll Bring
Qualifications
* Registered Nurse with current, unrestricted AHPRA registration
Experience
* Minimum 2 years' clinical experience (aged, palliative, or home care preferred)
* Proven ability to manage complex cases and deliver high-quality care
* Experience in grief counselling or palliative communication highly regarded
Skills & capabilities
* Excellent clinical assessment, mentoring, and decision-making skills
* Strong communication, resilience, and compassion in emotionally complex situations
* Knowledge of End of Life care, Advance Care Planning, and aged care standards
* Culturally competent and trauma-informed approach
