Responsibilities

3. Advocacy: Act as an advocate for participants, helping them navigate the NDIS system and access their rights and entitlements.
4. Collaboration: Work closely with a multidisciplinary team, including therapists, support workers, and external service providers, to ensure a holistic approach to service delivery.
5. Monitoring and Evaluation: Regularly review and monitor the effectiveness of support services, making adjustments as needed to ensure optimal outcomes for participants.
6. Documentation and Reporting: Maintain accurate and up-to-date records of participant progress, service delivery, and compliance with NDIS requirements.
7. Community Engagement: Foster relationships with local community organizations and resources to enhance service offerings and promote inclusion for individuals with disabilities.
Qualifications
* Relevant qualifications in social work, disability services, community services, or a related field.
* Experience in coordinating services for individuals with disabilities, preferably within the NDIS framework.
* Strong understanding of the NDIS, including its policies, procedures, and funding guidelines.
* Excellent verbal and written communication and interpersonal skills, with the ability to build rapport with clients, families, and stakeholders.
* Strong organizational skills and the ability to manage multiple cases effectively.
* Compassionate and empathetic approach to working with individuals with diverse needs.
