**Job Summary**
As a Disability Support Worker, you will provide essential assistance and support to individuals with disabilities, helping them achieve their personal goals and improve their quality of life. The role involves offering practical support, fostering independence, and promoting social inclusion for people with varying levels of physical, intellectual, or sensory disabilities as well as taking them to different appointments.
**Key Responsibilities**
- **Personal Care**: Assist clients with daily activities such as personal hygiene, dressing, grooming, and mobility, ensuring their comfort and dignity.
- **Support with Daily Living**: Aid clients with meal preparation, medication management, and household chores.
- **Community Participation**: Encourage and facilitate participation in social, recreational, and community activities, helping clients develop and maintain social relationships.
- **Skill Development**: Support clients in developing life skills, including budgeting, cooking, and personal safety.
- **Health and Well-being**: Monitor and report any changes in clients' health or behavior, ensuring appropriate responses and interventions.
- **Advocacy**: Advocate for clients' needs and rights, ensuring they receive appropriate services and support.
- **Documentation**: Maintain accurate and confidential records of client progress, incidents, and activities, in compliance with organizational and legal standards.
- **Collaboration**: Work closely with other professionals, including healthcare providers, therapists, and family members, to coordinate care and support plans.
**Qualifications**
- **Education**: Relevant qualification in disability support, social work, community services, or a related field is preferred (Minimum Certificate 3 Individual Support).
- **Experience**: Previous experience working with individuals with disabilities or in a similar role is advantageous.

**Certifications**
- First Aid and CPR certification.
- Valid driver's license + Reliable Vehicle

**Working Conditions**
- **Hours**:Split Shifts (Morning & Afternoon Shift)
- **Environment**: May involve working in various settings including clients' homes, community centers, or residential facilities.
- **Physical Demands**: May require lifting, supporting mobility, and other physical activities.
