Job Summary:
The successful candidate will provide expert clinical care and support to patients requiring palliative care, working closely with multidisciplinary teams to deliver high-quality patient outcomes.
Key Responsibilities:
* Provide specialist palliative care services to patients, including assessment, planning, implementation, and evaluation of care.
* Collaborate with healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
* Maintain accurate records and communicate effectively with patients, families, and healthcare teams to ensure seamless care delivery.
* Stay up-to-date with the latest research, guidelines, and best practices in palliative care to ensure the highest standard of care for patients.
* Contribute to quality improvement initiatives and participate in professional development opportunities to enhance skills and knowledge.
Requirements:
* Degree-qualified nurse with current registration and postgraduate qualification relevant to speciality.
* Minimum 3 years' experience working in palliative care or related field.
* Advanced level of knowledge and skill in palliative care, chronic disease management, and Aboriginal and Torres Strait Islander health.
* Effective communication and interpersonal skills, with ability to work collaboratively with diverse stakeholders.
* Ability to work autonomously and as part of a team, prioritizing tasks and managing workload effectively.
