Job Summary

As a Hospice Speech Language Pathologist, you will plan, organize, and deliver compassionate, specialized therapy services focused on comfort, communication, and quality of life. Essential Functions Patient Care
* Conduct evaluations and assess the home environment to determine equipment and speech/language needs.
* Identify functional speech and communication deficits; establish an individualized plan of care to improve or maintain essential function.
* Administer speech therapy programs using specialized techniques and equipment.
* Recommend communication devices or assistive aides as needed.
* Instruct patients, families, caregivers, and agency personnel on treatment plans, communication strategies, and safe care techniques.
* Collaborate closely with referring physicians and the interdisciplinary team to ensure comprehensive, coordinated care.
Communication & Documentation
* Prepare and submit clinical notes in alignment with organizational policy.
* Communicate ongoing patient needs, changes, and progress to physicians, caregivers, and team members.
* Participate in interdisciplinary group (IDG) meetings, in‐services, and other required team activities to support holistic care planning.
Additional Responsibilities
* Maintain clinical competency through continued education and adherence to current SLP practices and theory.
* Participate in peer consultation, orientation of hospice personnel, and quality performance improvement activities.
* When appropriate, supervise hospice aides performing tasks related to speech‐language therapy.
* Perform other duties necessary to support effective delivery of SLP services.
Qualifications
* Master's degree in Speech‐Language Pathology from an approved program, including required internship.
* Meets education and experience requirements for the Certificate of Clinical Competence (CCC) from ASHA.
* Certified and licensed to practice Speech‐Language Pathology in the state of practice.
* Current CPR certification.
* Licensed driver with an insured, reliable automobile meeting organizational requirements.
* Minimum of one year of clinical SLP experience.
* Understanding of hospice philosophy and comfort‐focused care.
