Working in clients' homes, you'll provide specialised equipment solutions and pressure care strategies. You'll also be responsible for:* Managing a wide variety of client presentations across multiple community aged care funding programs including **Home Care Packages, Commonwealth Home Support Program, Transitional Care Program and Palliative Care**.* Conducting in‐home assessments and developing tailored treatment plans to help clients meet their goals. This includes **simple and complex equipment prescription, major and minor home modifications, and client education**.* **Collaborating within a supportive statewide Occupational Therapy team of 70+ OTs** with decades of collective experience, as well as working locally with a diverse multidisciplinary team of nurses, care workers and support staff.At Blue Care, we encourage creativity, collaboration, and a positive team culture.
