Join Our Caring Team: Community Support Worker
Are you passionate about empowering people to live independently in their own homes? We're seeking a dedicated Community Support Worker to support individuals aged, frail, or with disabilities under the Consumer Directed Care model in Eyre and Far North LHN.
In this role, you will provide personalised services such as personal care, medication support, cleaning, meal preparation, transport, and social support. You'll work closely with consumers, respecting their choices and goals, as they actively manage their care and decide the services they receive.
